Official form from the Department of the Interior Bureau of Pensions. It is addressed to William Noyes and asks him to please answer several questions pertaining to his family. These questions include whether he is married, who his wife is and where they were married, what record he has of that, whether he was previously married, and whether he has any children. He lists his wife's name (Emily Maria Vinsonn) and the names and birth dates of his four children.
