Soldier's application for pension due to chronic illness as a result of war injuries. Appoints Joseph M. Maitland as his attorney to prosecute his claim. Signed by E. B. [Cundiff] and S.M. Woods as witnesses and a Mr. Ellis from Com[mon] Pleas Court (first name illegible).
