Form from the Army of the United States to certify discharge due to disability. It is filled in with information regarding William Wallace Noyes, who served in 2nd Lieutenant A. D. Beckwith's Company of the Second Vermont Regiment. According to the document, Noyes was drafted on July 21, 1863.
He was wounded in the Battle of St. Petersburg on April 2, 1865, in the lower third of his right tibia. He had been unfit to serve since, and was discharged with the notation that he was not fit to re-enlist at a later date.
The document is signed by someone whose first name is Warren but whose last name is difficult to read. He was surgeon in the army.
