Certificate from the State Board of Education and Health certifying that Eugene Williams "attained the five point standard" in physical education from Lucy Addision High School. It is signed by the division superintendent, state superintendent of public instruction, and the state health commissioner. There is also a gold seal, which says indicates the high five standard.

The copyright law of the United States (title 17, United States Code) governs the making of photocopies or other reproductions of copyrighted material.
