Captain Ethan A. Jenks signs and completes docket information. Inventory includes bayonet scabbards, Enfield 57.40 Caliber Rifle Muskets, ammunition for the muskets, cartridge boxes, gun slings, and waist belts. These items were received by Captain David R. Kenyon on 10 March 1863. They were approved for distribution by the Regimental Quartermaster, Lieutenant James N. Potter on 1 March 1863. The document lists lost items and notes the number of each item to be accounted for upon return of the Company. This document is marked as a report for the quarter ending 31 March 1863. Appears to be the same information as found on 02750.432, though the printed form is formatted slightly differently. This document has a heading at the top saying it is "Form No. 1.--(a.) For the use of Infantry and Heavy Artillery". Possibly filled out a second time to account for the updated form, possibly just a second copy for Captain Jenks to retain for his files.
