Appoints James Elliot as a major and aide de camp for Major General Simon Elliot. Signed by James Sullivan as Governor of Massachusetts. Also signed by Jonathan L. Austin as "Secretary." Docket on verso is signed by Simon Elliot and reads "James H. Elliot took and subscribed the oaths and declarations perscribed by the Constitution of the Commonwealth of Massachusetts previous to his entering upon the duties assigned him by the within Commission."
