She began to fill it out after her sons' death but appears to not have finished or had it notarized. She details her relationship to him and lists his surviving relatives. On the back is printed penalties for insurance fraud.

The writing on the bottom of the document implies that this eulogy was given at the First Baptist Church of Roanoke, Virginia.
