Convenes a Board of Survey consisting of Captain Edward M. Carey, 12th Regiment Ohio Volunteer Infantry, Captain Thomas Hayes, 30th Ohio, and 1st Lieutenant Russell Hasting, 23rd Ohio. The board was to determine the property of the 1st Provisional Brigade lost or destroyed on the march from "Adairs" at the mouth of the East River, for which Lieutenant Robert S. Gardner, Acting Assistant Quarter Master, was responsible. Also includes the property of the 12th Ohio regiment lost or destroyed for which Sergeant Jonathan C. Wallace was responsible. By order of Colonel Scammon of the 1st brigade, and witnessed by Robert P. Kennedy, the Acting Assistant Adjutant General. Pages two and three contain the board's findings. Signed on page three by Carey, Hayes, and Hastings. States that Wallace is not responsible for the loss of stores because of lack of transportation. Written from Headquarters of the 1st Brigade at Flat Top Mountain. With autograph endorsement signed on verso by Scammon stating the report was approved.
