Attests that doctor Jonathan Horton served in as a surgeon in the military, even though his name does not appear in hospital books or on rolls. States that his widow is therefore entitled to his pension, since he died of a "putrid fever while Officiating in the public service." Noted as "duplicate." Docket is on a piece of paper separated from the document.
Jonathan Horton was a surgeon in the New Jersey Militia and hospital surgeon for the Northern Department.
