Full title: "Alphabetical Index to Places of Internment of Deceased Union Soldiers in the Various States and Territories, as Specified in Roll of Honor Nos. I-XIII, Being Those Issued from the Quartermaster General's Office, Between June 15, 1865, and August 27, 1867." Printed by the Government Printing Office. Report prepared by Brevet Brigadier General Perry as Quartermaster of the United States. The report was submitted to Acting Quartermaster General D.H. Rucker. Contains a correction to the previously published Rolls of Honor - numbers 1-13 - covering 15 June 1865 to 27 August 1867. The alphabetical index is divided into 42 different states and territories. Each cemetery in the state or territory is then listed in alphabetical order. Provides a correction for the number of graves and gives pertinent volume and page number of the Roll of Honor where the mistake was made. Many of the graves referenced are for "Colored" soldiers. On page 10, Topeka was added in pen under the Kansas heading. Page titled "Memorandum" has pencil additions for volumes 14-17 from 20 February 1868 to 9 June 1869, as well as pencil check marks next to various volumes. Originally wrappers and stab-stich binding. Bottom of back cover has a tear a piece missing.
