Form from the Army of the United States to certify discharge due to disability. It is filled in with information regarding William Wallace Noyes, who served in 2nd Lieutenant A. D. Beckwith's Company of the Second Vermont Regiment. According to the document, Noyes was drafted on July 21, 1863. He was wounded in the Battle of St. Petersburg on April 2, 1865, in the lower third of his right tibia. Lovell enlists himself in the British army from the Province of Massachusetts Bay, noting that he received five dollars as bounty money from Captain Timothy Hamant. Indicates that he is joining a mission for "the total Reduction of Canada." Lovell died on 26 October 1760 at Ticonderoga, New York.

Burns discusses his settlement on Samuel Waldo's Muscongus Patent (also known as the Waldo or Linconshire Patent, land in present-day Maine). Discusses improvements Burns made to a stone house, and mentions his black cattle. Burns was given command of a company of soldiers by Governor [William] Shirley, after which he and his family moved to a settlement of 40 or 50 "Protestant Familys from Germany" on the Madomock [Medomak] River.
