The first page is acknowledgment that Eugene Williams returned the funds and is signed by S.R. Weaver. The reverse contains instructions for filling out the form. The third page has two forms, one detailing the funds "intrusted" to Eugene Williams (signed by S.R. Weaver) and the other giving Eugene Williams pecuniary responsibilty. The reverse contains instructions for filling out the forms on page three.
